The Case Manager, under the general supervision of the Behavioral Health Manager or Director of Clinical Services, serves as main point of contact for Bakersfield American Indian Health Project (BAIHP) clinical services. The Case Manager is responsible for the clinical administrative functions of patient intake scheduling, registration, and clinic-based activities required to ensure accurate billing and claims processes. The Case Manager also ensures duties are accomplished consistently with BAIHP’s policies, procedures, and HIPAA requirements. The Case Manager provides case management services while assisting the behavioral health professional staff in the development and delivery of services to those entering the behavioral health system. The Case Manager participates in group counseling programs; provides homeless tracking and data entry, functions as a direct support to all treatment support staff for the clients and maintains liaison with community agencies and groups and completes documentation in the Electronic Health Record (EHR) system. Emphasis is on the interdisciplinary team approach as a problem-solving process in providing comprehensive care to clients and their families. This position must have experience providing culturally competent services to clients including diagnostic evaluations, Enhanced Care Management and Community Supports or equivalent to CalAIM services. Assists with justice involved coordination, school-based contact and programming, therapy and case management services for individuals, couples, and families. The Case Manager will provide behavioral health care with dignity for all, with the understanding clients have the right to quality behavioral health care including participation in planning and wellness and recovery action plan services.
