The primary purpose of the Registered Nurse (RN) is to determine, assess, and plan health and case management services for patients enrolling to the clinic who also are seeking enhanced care management, supportive services, or any other eligible services offered by the organization. The RN ensures that services promote the early detection of diseases and or illnesses. The RN serves as a central point of contact for patients and provides services that emphasize health care check and screening that are preventive in nature. The RN is part of a multi-disciplinary team working together to establish and implement a case plan for each client: monitor health care goals and outcomes; deliver and meet grant-based healthcare objectives and milestones, to effect overall wellness support programs funded by federal, state, and local funding sources. This may include providing safe in-home assessments, screenings, and care. Participation in activities weekdays and weekends at community-based outreach events is a standard practice of the entire medical crew.
